| SUPPORTED
LIVING SERVICE
• This service is funded through the Ministry of Health to
assist the person to live in their own home, in a situation of their
own choosing and to establish community lives that they consider
to be personally rewarding.
• This means that each person has a unique living and support
arrangement. Our main role is to facilitate and develop a personalised
support system around each person’s needs and aspirations,
focusing on community belonging and making the best use of natural
and community supports.
• Eligibility for this service is assessed through the local
Needs Assessment and Service Coordination Service.
VOCATIONAL
SERVICES
• These
services are available for people who require assistance to participate
in the community. Assistance is given to support participation in
a broad range of activities, ranging from paid work to non-work
options including group membership, learning opportunities and community
contribution.
• Our focus is on assisting the person to identify possible
opportunities for connecting and contributing to the community and
to relationships in ways that are personally meaningful. We are
funded for this work through a range of Ministry of Social Development
