OUR SERVICES AND SUPPORTS


We are funded by a variety of government contracts to assist people to live, work and participate in their community.
 

SUPPORTED LIVING SERVICE

• This service is funded through the Ministry of Health to assist the person to live in their own home, in a situation of their own choosing and to establish community lives that they consider to be personally rewarding.
• This means that each person has a unique living and support arrangement. Our main role is to facilitate and develop a personalised support system around each person’s needs and aspirations, focusing on community belonging and making the best use of natural and community supports.
• Eligibility for this service is assessed through the local Needs Assessment and Service Coordination Service.

VOCATIONAL SERVICES

These services are available for people who require assistance to participate in the community. Assistance is given to support participation in a broad range of activities, ranging from paid work to non-work options including group membership, learning opportunities and community contribution.
• Our focus is on assisting the person to identify possible opportunities for connecting and contributing to the community and to relationships in ways that are personally meaningful. We are funded for this work through a range of Ministry of Social Development Contracts:

Community Participation Services:
This service is for people with a permanent disability aged 16 years or more who would like to increase their participation in the community and in work.

Very High Needs Individual Contracts:
This service is available for young adults who have left school and who have been assessed by the Ongoing and Reviewable Resource Scheme as having very high needs.

Transition Services:
This service is funded for students aged between 16 and 21, funded by the Ongoing and Reviewable Resource Scheme (ORRS) and are in their final year of school or tertiary education.
